How to Order

Mira Dx has made it easy for physicians to order testing when appropriate.  There are two methods that can be used to order from us outlined below:

1. Blood Sample:  Patients may have their blood drawn at a local hospital laboratory or reference lab.  The preferred sample is 10 ml in a yellow top ACDA tube.  Please ship sample within 48 hours of draw.  Refrigerate until shipped. Please contact Mira Dx at (203) 789-9000 for alternative tube types or complete the online form to order Mira Dx shipping kits with tube at no charge.

2. Saliva Sample:  Mira Dx provides an Oragene 10 ml saliva kit at no charge to the patient. Please note specimen requirements. Contact Mira Dx at (203) 789-9000 or complete the online form to order the Mira Dx saliva collection kit.

Please include a copy of the test requisition (pdf) with each order.

New York Samples:  Mira Dx can accept samples from NY state as long as the sample is accompanied by the following form. New York State Non-Permitted Laboratory Request Form (pdf).
 
International Samples:  Mira Dx prefers all international samples to come in via the Oragene saliva kit (1ml or 2ml).  To discuss other sample submission options, please call Mira Dx and ask to speak to Dr. Mark Rabin at 001 (203) 789-9000 x204.
