How to Order PreOvar®
PreOvar® must be ordered by a healthcare provider. PreOvar® testing can be performed on a blood or saliva sample. Test kits may be ordered by a physicians office free of charge by calling Mira Dx® at (203) 789-9000 or by completing the online form.
The PreOvar® test kit includes a Test Requisition Form, Informed Consent Form, Family History Questionnaire, detailed instructions, and everything necessary for return shipping to Mira Dx®.

Blood Sample: The preferred sample is 10 ml in a yellow top ACDA tube (included in PreOvar® test kit). Please ship sample within 48 hours of draw. Refrigerate until shipped. Please contact Mira Dx® at (203) 789-9000 or complete the online form to order Mira Dx® shipping kits with tube at no charge.
Saliva Sample: Mira Dx® provides an Oragene 10 ml saliva kit at no charge. Please note specimen requirements. Contact Mira Dx® at (203) 789-9000 or complete the online form to order the Mira Dx® saliva collection kit.
